    Physical Activity Questionnaire
  1. If you answer yes to any of the following questions, please provide a written medical consent prior to participation:
  2. Has your doctor ever said that you have a heart condition and you should only do physical activity recommended by a doctor?
  3. Do you feel pain in your chest when you do physical activity?
  4. In the past month, have you had chest pain when doing physical activity?
  5. Do you lose balance because of dizziness or do you ever lose consciousness?
  6. Do you have a bone or joint problem that could be made worse by the change in your physical activity?
  7. Is your doctor currently prescribing drugs (i.e. water pills) for your heart condition?
  8. Do you know of any other reason why you should not do physical activity?

FITNESS CENTRE GUIDELINES

  • Please sign in before commencing your workout.
  • Guests must be introduced to the fitness centre upon payment of a $10 guest fee ( chit to be signed in the locker room). The same guest may not be introduced more than once per month. All guests must sign the approved waiver found in the fitness centre.
  • Members and guests 13-16 years of age may use the facility accompanied by a parent at all times. Guests may be introduced by members 18 years of age or older.
  • Dress code includes shorts and t-shirt, or workout attire and comfortable training shoes. It is recommended that a tracksuit or sweatshirt be worn to and from the fitness centre.
  • No food or drinks are permitted in the fitness centre. Water in plastic bottles is allowed.
  • Please return the weights and equipment to their proper storage area.
  • Equipment orientations or personal training sessions are recommended prior to the use of the equipment.
  • Lockers are for daily usage only.
  • All other clubhouse rules apply.
  • NOTE: There is a 30 minute time limit for the treadmills, elliptical trainers and stationary bikes.
